Medical Diagnosis please?

My friend had this weird rash on her right shin. She had it treated for a while and just as it was about to get better, she stopped treatment. Now in the past months it has worsen a lot. It went from a small red spot rash to a blacken sunken area on her leg. It's like the bone is nearly visible.
