Where do Souls come from?

Does God create them "on the fly" or is there a big jar of them that he has pre-made since the time of creation? When does a soul get attached to a human being (conception, birth, consciousness?) and when does it leave a human being? (when it's sold, or 10 minutes after brain death, or...?)
